	The aim of this form is to :- Lighten the workload of voluntary staff Simplify the chances of local musicians performing at a Frakture Big Band event by providing a clear process Remove the need for several phone calls & emails How to use this form :- Fill it in when you have been informed of future sessions via email, text or conversation. If there is a choice of events, complete a separate form for each event please. All requests will be acknowledge as soon as possible.
