* Fill it in when you have been informed of available slots via email, text or conversation.
* If there is a choice of dates enter your first choice date in the form and a second choice within 'Notes to promoter'.
* DO NOT fill in this form if you have neither a venue or date given to you by Frakture.
* All requests will receive a reply within 14 days.
